What is Fistula Surgery?

An anal fistula is a small tunnel that develops between the end of the bowel and the skin near the anus. It is usually caused by an infection near the anus, which results in a collection of pus (abscess) in the nearby tissue. If left untreated, the abscess can burst, leading to the formation of a fistula. Most anal fistulas require surgery as they rarely heal if left untreated. The main surgical options for treating an anal fistula include fistulotomy, seton procedures, and bioprosthetic plug insertion. A fistulotomy involves cutting open the whole length of the fistula so it heals. This procedure is usually performed under general anesthesia and requires a few weeks of recovery time. A seton procedure involves placing a piece of surgical thread called a seton in the fistula to help it heal. The seton is left in place for several weeks or months until the fistula has healed. Recovery from fistula surgery usually takes several weeks, and patients may experience some pain and discomfort during this time. Pain medication can be prescribed to manage the pain. Patients should also follow a healthy diet and avoid constipation to promote healing. It is important to avoid strenuous activities during the recovery period to prevent complications.

How You Prepare for Fistula Surgery

Proper preparation for anal fistula surgery helps ensure better outcomes and smoother recovery. Follow these guidelines as directed by your colorectal surgeon.

Food and Medicines

  • Follow fasting instructions carefully. Typically, avoid solid foods for 8 hours and clear liquids for 2-4 hours before surgery.
  • You may need to perform bowel preparation the day before surgery. This might include clear liquid diet, laxatives, or enemas as prescribed by your doctor.
  • Inform your surgeon about all medications and supplements you take. Blood thinners, aspirin, and anti-inflammatory medications may need to be temporarily stopped 5-7 days before surgery.
  • Take any prescribed antibiotics exactly as directed before the procedure.

What to Expect

During & After Surgery
  • Fistula surgery is typically performed under general anesthesia or spinal anesthesia.
  • The procedure usually takes between 30-60 minutes, depending on the complexity of the fistula.
  • Most patients go home the same day, though some complex procedures might require an overnight stay.
  • Some discomfort and drainage from the surgical site is normal during recovery.
  • Recovery time varies from 2-8 weeks depending on the surgical technique used and the complexity of your fistula.
  • Follow-up appointments are essential to ensure proper healing and prevent recurrence.

Frequentlly Asked Questions About Surgery Second Opinions

Get expert answers about our second opinion process for surgery, specialist consultations, and how our medical experts can help confirm your diagnosis and treatment plan.

Q: What information should I provide for a fistula surgery?

A: Include your MRI or fistulogram results, history of symptoms, previous surgeries (if any), current medications, and surgeon’s treatment plan.

Q: Is surgery the only treatment for fistula?

A: Surgery is typically required, but approaches differ. Options include fistulotomy, seton placement, laser ablation, and newer minimally invasive techniques.

Q: Are there better techniques for fistula surgery to improve outcomes?

A: Yes. Advanced approaches such as sphincter-sparing or flap techniques may reduce the risk of incontinence and promote better healing in selected cases.

Q: What happens if fistula treatment is delayed?

A: Delays can lead to chronic infections, recurrent abscesses, or the development of more complex fistulas, making treatment more difficult later.

Q: Can a fistula come back after surgery?

A: Yes. Recurrence is possible, particularly in complex cases or if underlying conditions like Crohn’s disease are present. Proper evaluation and technique help reduce this risk.

Q: Is laser or minimally invasive surgery suitable for all fistulas?

A: Not always. These methods work best for certain fistula types. Success depends on the fistula’s size, location, and presence of infection.
